The Transition Centre Kingston, located at 496 Discovery Avenue within the Canadian Forces Base (CFB) Kingston area, is a dedicated facility established to support Canadian Armed Forces (CAF) members, veterans, and their families. As part of the Canadian Armed Forces Transition Group, this centre provides professional, personalized, and standardized services to assist members as they navigate the complex process of transitioning from active military service to civilian life. The centre focuses particularly on those who are ill or injured, ensuring they receive the specialized care and administrative support required for a successful reintegration.

Integrated Services and Support

Operating as a one-stop resource, the Transition Centre integrates various partners to offer comprehensive support:

• Transition Services: Staff assist members with release administration, career transition workshops, and developing personalized transition plans that address health, finances, and housing.
• Casualty Support: Dedicated case managers work with ill and injured personnel to coordinate medical care, return-to-duty programs, or medical release procedures.
• Veterans Affairs Canada (VAC): VAC staff are co-located at the centre to provide immediate information on benefits, disability claims, and rehabilitation programs, ensuring a seamless handover of care.
• Family Services: Recognizing the impact of service on the home front, the centre connects families with the Military Family Resource Centre (MFRC) and other support networks.
• Financial & Insurance: Guidance is available regarding SISIP Financial services and other insurance matters relevant to releasing members.

The centre is open to all Regular and Reserve Force members, as well as veterans and their families.
